A Granite VNA Social Worker/Notary Public is available to connect with you to discuss your end-of-life wishes, review your Advance Directives to make sure it reflects your decisions regarding future medical care, assist with filling out your Durable Power of Attorney for Healthcare and/or Living Will, provide information about resources and services that may benefit you or a family member.
